1. Right Patient;
Ensure the medication is given to the correct individual by checking their name, ID band, date of birth, or medical record number.
2. Right Medication;
Verify the drug name and form against the prescription or doctor's order to ensure it matches exactly—no look-alike or sound-alike confusion.
3. Right Dose;
Confirm the correct dosage—check calculations, units (mg, mL), and double-check high-risk medications.
4. Right Route;
Administer the drug using the correct method—oral, IV, IM, subcutaneous, etc.—as prescribed.
5. Right Time;
Give the medication at the correct time and frequency (e.g., once daily, every 6 hours), and note any food or timing considerations.
